EVANSVILLE, Ind. (WFIE) - Officials with Ascension Health Care says some patient information was likely stolen from a business partner.

According to a news release, officials learned on December 5 information belonging to patients may have been involved in a potential security incident.

The incident affected 5 states, including Indiana.

After an investigation, it was determined the information was likely stolen from the former business partner due to a vulnerability in third-party software.

Ascension officials say they have reviewed their processes and are working to implement enhanced measures to prevent similar incidents from occurring in the future.

The investigation determined that the personal information involved in this incident included demographic information, such as name, address, phone number, email address, date of birth, race, gender, and Social Security number, as well as clinical information related to an inpatient visit.

Officials say the exact type of information involved depends on the individual.

Ascension is offering two years of complimentary credit monitoring and identity theft protection services to those impacted.

A dedicated call center for questions has been set up at (866) 408-3556, Monday through Friday between 8:00 a.m. and 5:30 p.m. Central Time.
